As part of our role in decreasing the risk and spread of coronavirus, we are asking you to do the following:

• Please wear your own facemask to your appointment or procedure. You will be expected to wear a facemask for the time you are at our office.
• In order to decrease exposure for you as well as our staff, you will be the only person permitted into the office. Your ride (driver) will be asked to remain in their car unless they are needed to assist you during your visit.
• When you arrive for your appointment or procedure you will have your temperature taken and you will be asked coronavirus screening questions.
• Please maintain social distancing while in our facility as much as possible.
• Please do not come to the office if you are ill the day of your appointment.Call the office to reschedule at 937-320-5050.

We have telehealth capabilities to provide virtual office visits and consultations.
